OPTOCS-AI: Optimizing Pharmacy Transition of Care Services Using Artificial Intelligence

The proposal's long term goal is to develop a TOC visualization tool using cutting-edge AI methods. The overall objective is to first identify MRC patterns during admission and discharge and their association with clinical outcomes. 

Second is to predict high-risk patient profiles for readmission. The rationale for the project is to reveal patterns and determinants of higher MRC scores to improve the safety and appropriate use of medications in accordance with best practices guidelines.
